  • Free immunizations to any child from birth through 18 years of age if VFC eligible (Vaccines for Children Program).
  • Proof of Immunization record, such as the 680 form that is needed for school entry and day care.
  • Educational materials on immunizations.
  • Selected immunization outreach events for underserved populations.
  • School-age immunizations in selected schools.
  • Participation in selected community health fairs dependent upon availability.
  • Coordination of mass immunization projects when necessary.
  • On-site education to groups in our community based on availability.
  • On-site immunizations throughout the community to senior residents of group homes, churches, and organizations based on availability.
  • Ongoing tracking of infant immunizations to assess completion of the primary immunization series.
