Eligibility

To be eligible you must:

  • Be pregnant (proof of pregnancy required, including the number of babies expected, and the estimated due date)
  • Be a US Citizen (documentation required)
  • Be a Florida resident
  • Meet the income eligibility requirements.
  • Fill out and mail or fax a completed application to the Department of Children and Families
  • Provide all the necessary documentation

Your income must meet the following guidelines:

If your household includes more than 12 people, please add $525 for each additional person.

A Mom Care advisor will also call or send a letter to you when your Medicaid Insurance Application has been approved. Your Medicaid information will be given to you on your request. If you attend an appointment at the Orange County Health Department's Eligibility Office you will also be offered a clinic appointment for your first visit. Should you have any concerns, you can follow up with your Mom Care Advisor.

When you contact your Maternity Care Advisor you will receive information about what prenatal care providers including doctors and midwives, accept your coverage. If you have any questions, please call 407-836-9261.
