The Vital Statistics location at the Orange County Tax Collector Office in Clarcona Ocoee will no longer be operating at this site after 11 a.m. on Friday, April 25, 2014. Clients can still obtain Vital Certificate services at our two other locations at the Church Street Vitals Office and the Orlando Health Campus.
To make it more convenient for you, the Orange County Health Department, in conjunction with the Department of Orange County Tax Collector, is offering one stop shopping when getting or renewing your driver's license or ID card at the Orange County Tax Collector's Agency located at 4101 Clarcona-Ocoee Road, Orlando, FL 32810. A new federal law requires you to have a certified copy of your birth certificate to renew your license...and you can get both easily and quickly at the same place. Many people don't have a certified copy of their birth certificate.
A birth certificate with your footprint, or a plain black and white copy, is not certified birth certificate. A certified birth certificate only costs $15.00.
Only parents, legal guardians, or the person whose name appears on the certificates, if they are at least 18 years of age, can receive a birth certificate.
A computer generated birth certificate that contains a State File Number is accepted as the official certification of birth and is recognized as such by all agencies. A computer copy of birth certificates of all Florida births from 1917 to the present are available for a cost of $15.00 for each certified copy. Additional computer copies for the same person are $8.00 each.
A certified copy of your birth certificate is needed for many things: obtaining a driver’s license and passport, applying for social security and social service benefits, and entering school or the military. Your Birth Certificate…You Never Know When You Will Need It!

A copy of your driver's license or ID Card must also be forwarded with the application form. Mail application to 807 W Church Street, Orlando, FL 32805-2211 (please send a self-addressed stamped-envelope to return your certificate to you).
Please make money orders or cashier's checks payable to Vital Statistics. Personal checks are not accepted at this time.
BY CREDIT CARD ONLINE OR FAX - you can go to VitalChek.Com or call (866) 830-1906. Your certificate can be received by regular mail or express shipping by the next day in many cases.
