Medical Records Department will Temporarly move from the 1st floor to the Auditorium on the 4th floor (Lilac Room).
The move will be effective from May 16, 2013 thru May 29, 2013.
The Medical Records department is located on the 1st floor of the Central Orange County Health Department:
832 W. Central Blvd.,
If you are a patient and need to request a copy of your medical records please fill out the authorization form below and bring or fax it with a copy of your I.D. to the Medical Records Department.
*If you are unable to pick up your records, you can send a representative with a notarized letter from you stating they have permission to request the records along with a copy of your picture I.D. and their picture I.D.
If you are an organization or government agency needing to request medical records for a client, please fill out and fax the form below.
