Commercial Systems- New

Onsite Sewage Treatment and Disposal Systems are regulated by the Florida Department of Health and the Department of Environmental Protection under Chapter 64E-6 of the Florida Administrative Code.

The following forms are required for a New Permit:

• DH4015-1 Construction Permit Application
• DH4015-3 Soil Evaluation Form
• Site Information Documentation Form
• DH4081 Operating Permit Application
• Survey to Amend Annual Operating Permit

To apply: Download an Application Packet (PDF 531kb) and submit all required items with permit fee to:

• Application submitted with a Site Evaluation (conducted by certified personnel) - $294.00
• Application submitted without a Site Evaluation (to be conducted by Health Department Staff) - $409.00
