What are the hours of operation?
Hours of operation for our main office located at 832 W. Central Blvd:

Is there anywhere else in Orange County I can apply in person for a birth certificate?

If I come to your office during your hours of operation, can I receive my birth certificate
right away?
In most cases you will only have to wait 10 to 15 minutes to receive your birth certificate.
Do I need identification?
Yes, you will need an up-to-date identification; preferably a state driver’s license or state identification card.
What years do you have certificates available for Orange County births?
Computers copies are available from 1930 to the present for anyone born in the state of Florida.
Book copies are available for babies born in Orange County from 2006 to the present.
What years do you have certificates available for Orange County deaths?
We have death certificates for the years 2006 to the present.

Does the birth application need to be notarized before mailing it to your office?
There is no need to have the birth application notarized. Just send us a copy of your up-to-date state driver’s license or state identification card, along with your application and a self-addressed stamped envelope.

Are there any restrictions on who can request a birth certificate?
Yes, there is an eligibility process.
Eligibility:
Certificates can be issued only to the person named on the record, if of legal age (18); parents, guardian, or their legal representative, or by court order. In the case of a deceased registrant, upon receipt of the death certificate of the decedent, a certification of the birth certificate can be issued to the spouse; child; grandchild; sibling; if of legal age; or to their legal representative. If not any of the above, an affidavit to release birth certificate will have to be obtained by the parent and notarized giving permission to get the birth certificate.
Are there any restrictions on who can request a death certificate?
Yes, there is an eligibility process.
Eligibility:
Death records that list the cause of death information may only be issued to the following individuals: the decedent’s spouse or parents; to the decedent’s child; grandchild; or siblings, if of legal age. Any person who provides a will, insurance policy or other documents that demonstrates interest in the estate of the decedent, or to any person who provides documentation that he or she is acting on the behalf of any of the above named persons may request a death certificate. All requests for a death certificate, that lists the cause of death, must include a signature of the applicant (indicating qualifying eligibility), or provide a notarized Affidavit to Release Cause of Death Information (DH Form1959), which is available upon request.
