REPAIR VERIFICATION REQUIREMENTS

The following forms are required for repair permit application.

  1. Application DH 4015 (PDF 23kb)
  2. Existing and Repair Certification DH 4015 (PDF 16kb)
  3. Site Information Form (PDF 32kb)
  4. Pump Out Form DH 4015 (PDF 16kb)
  5. Soil Evaluation Form DH 4015 (PDF 15kb)
  6. Credit Card Authorization Form (PDF 47kb)
  7. Soil Evaluation Report by GEO memo (PDF 44kb)

The following documentation is required for repair permit application.

  1. Type of Establishment, office space square footage, number of occupants.
  2. Section, Township and Range.
  3. Specific directions to property.
  4. Type of water system utilities on property.
  5. Date of initial system construction prior to 1972, 1983 or after (if well or surface water setback apparent).  [64E-6.001(4)]
  6. Quantity and type of waste being discharged. [64E-6.007(1)(a)]
  7. Zoning classification.
  8. Certification of tank size and integrity from licensed Septic Contractor. [64E-6.001(4)]
  9. Mark the location of the 4 corners of the drain field.
  10. Floor plan(s). (If house is being modified)
  11. Water flow records for the previous 18 months (for commercial systems).
Site Plan Information Required:    [64E-6.001(4)]
  1. Lot boundaries with dimensions.
  2. Existing system configuration, tank and drain field size, tank inlet elevation, and location on the property.
  3. The building location.
  4. Utility or drainage easements.
  5. Obstructed areas (walkways, driveways, large trees, etc.).
  6. Location of private or public wells on property.
  7. Location of wells and onsite sewage disposal systems on adjoining property.
  8. Location of surface water bodies, storm water systems.
  9. Location of potable water and non-potable lines within the existing drain field
  10. area.
  11. General slope of the property.
  12. Unusual site conditions which may influence the system design such as roof drains, patios, decks, swimming pools or parking areas.
  13. A brief description of the nature of the occurring failure.
  14. Need to obtain an Onsite Sewage Disposal Operating Permit (if applicable) [64E-6.003(5)]

Application for a repair permit should be submitted with a $300.00 permit fee to:
