All change of ownership or new bars that do not serve food, assisted living facilities, schools, and childcare centers must get approval for the Department of Health before operating. Health, Safety, and Sanitation requirements must be met in order to obtain approval from the Division of Environmental Health. The online forms are located below for the requirements for each type of facility.

Address all payments and correspondence to the Orange County Health Department, Attn: Division of Environmental Health, located at 800 N. Mercy Dr., Suite 1, Orlando, FL 32808. The Division of Environmental Health can take credit cards utilizing the credit card authorization form.
