Healthcare Provider and Lab Reporting

Orange County Health Department Epidemiology accepts reports of communicable diseases as listed on the current Reportable Diseases list, from healthcare providers, hospitals and laboratories. A “Report of Communicable Disease“ form (PDF 26kb) can be obtained by calling 407-858-1420. The form should be printed and completed in its entirety in order for immediate and accurate case follow-up. Questions regarding the form can be answered by calling 407-858-1420.
