Vision: Leading the way to providing the community HIV medical care, medications adherence education and counseling, peer education and support of excellence. Mission: The mission of the C.H.E.E.R.S. Program is to empower and support people infected and affected by HIV/AIDS to improve their health, manage their illness and promote the quality of their lives through education, advocacy and greater participation in decision making. The aim of improvement of the HIV/AIDS care delivery process in the community health system must be achieved through unity, respect for diversity, communication, team work and creativity. The C.H.E.E.R.S Program provides HIV medications adherence education, counseling and peer support to people living with HIV/AIDS in the community. Recognizing the complexity of the disease and the importance of patient readiness and optimal adherence to antiretroviral medications is a key process in the prevention of the development of resistance to current medications and the loss of future treatment options. The C.H.E.E.R.S Program works hand-in-hand with the Orange County Health Department Immunology clinic staff providing a comprehensive and integrated model of HIV care that includes: |
